По данным Евро­пей­ско­го респираторного общества,  до 30% обращений к врачу общей практики так или иначе связаны с развитием кашля в ночное время. В норме кашель выполняет защитную функцию, способствуя выведению из дыхательных путей секрета, инородных тел и раздражающих веществ. Кашель, в особенности хронический, сухой существенно снижает качество жизни пациентов, нарушая сон, физическую и интеллектуальную активность.

Кашель - не самостоятельное заболевание, а клинический симптом (нередко единственный) какого-либо заболевания или патологического состояния, поэтому, целью терапевтических мероприятий является устранение причины кашля. При установлении причины возникновения кашля в первую очередь должно проводиться этиотропное или патогенетическое лечение основного заболевания. Как правило, при эффективном лечении болезни, сопровождающейся кашлем, он также исчезает. К сожалению, в ряде случаев причину кашля установить не удается: даже в странах с высоким уровнем диагностики пациенты, страдающие хроническим кашлем неясной этиологии, составляют от 10 до 38% и лечатся, как правило, симптоматически c использованием противокашлевых препаратов.

Использование противокашлевых ЛС, преимущественно неопиоидных, может быть рекомендовано больным  с сухим, раздражающим кашлем (как острым, так и хроническим). Однако, следует учитывать, что при использовании данных ЛС могут наблюдаться такие побочные эффекты, как сонливость, заторможенность,  гастроинтестинальные и кардиоваскулярные нарушения. Поэтому при лечении хронического, сухого кашля целесообразным является использовать противокашлевые у которых отсутствуют вышеуказанные побочные эффекты. Одним из таких новых противокашлевых препаратов при лечении сухого кашля, является левоклоперастин.  

Левоклоперастин (Privituss) - новое противокашлевое средство, не вызывает зависимости или привыкания, не действует на кардиоваскулярные  и гастроинтестинальные функции, не вызывает сонливости. Левоклоперастин обладает -противокашлевым, бронхолитическим, противовоспалительным действиями. Левоклоперастин благодаря двойному механизму действия, воздействует на бульбарный центр кашля и на периферические рецепторы в трахеобронхиальном тракте. Левоклоперастин действует селективно на кашлевой центр, для избежания центральных побочных эффектов, напр. седации. Уменьшение центральных побочных эффектов левоклоперастина обусловлено специфической стереоизомерной структурой. Левоклоперастин даже в дозе 45 раз превышающей терапевтическую не вызывает клинически выраженную седацию.

Согласно клиническим исследованиям, левоклоперастин быстрее начинает действие и эффективно снижает частоту и интенсивность кашля, по сравнению с DL-клоперастином, кодеином и леводропропизин.

Фармакологические эффекты левоклоперастина были подтверждены в крупномасштабных клинических испытаниях, 10 испытаний показали, что левоклоперастин вызвал быстрый эффект после первого дня лечения. У детей, улучшение качества сна привели к значительному снижению раздражительности и к улучшению качества их жизни. Levocloperastine быстро снизил интенсивность и частоту  кашля, ночные пробуждения, одышку  по сравнению с исходным, без клинически значимых побочных эффектов на дыхательные или кардиологические функции.  Не выявлено центральных побочных эффектов с levocloperastine, в то время как levodropropizine, кодеин и DL-cloperastine вызвали сонливость.

Хороший профиль переносимости levocloperastine и отсутствие лекарственных взаимодействий делают его приемлемым при лечении пациентов, получающих комбинированную терапию (антибактериальные средства, нестероидные противовоспалительные препараты, кортикостероиды).

Исследования, проведенные в рамках программы клинических исследований для levocloperastine показали, что это противокашлевое средство является эффективным и хорошо переносимым при лечении кашля связанного с рядом острых и хронических респираторных заболеваний у пациентов всех возрастов.

Учитывая вышеуказанное, целью нашего исследования являлось оценка клинической эффективности  левоклоперастина  при сухом кашле различной этиологии. С этой целью,  в городе Баку обследовано 60 пациентов ( дети =30,  взрослые =30).  В обследуемую группу вошли больные со следующими диагнозами:

ларинготрахеит, бронхиальная астма (кашлевой вариант ), коклюш,  бронхит,  ОРВИ, плеврит.  Сбор анамнеза, для выявления частоты ночных и дневных приступов кашля, продолжительности кашля,  проводили с помощью специального опросника. Обследованной группе больных назначался препарат с составом левоклоперастин Privituss (единственный препаратом с данным составом) в течение 7-10 дней в соответствующей возрастной дозировке. 

Результаты клинического исследования по оценке клинической эффективности препарата Privituss показало следующее;

  • 1. Privituss обладает быстрым началом действия- противокашлевой эффект наблюдается с первых дней лечения у пациентов всех возрастов.
  • 2. Privituss быстро и значительно снижает продолжительность и частоту кашля, снижает пробуждения в ночное время.
  • 3. Privituss эффективная альтернатива применяемым противокашлевым средствам при лечении сухого кашля с дополнительными преимуществами: более быстрое начало действия и лучшая переносимость во всех группах пациентов

Современное направление в противокашлевой терапии.

Кашель является одной из частых жалоб пациентов с которой больные обращаются к врачу. Общепринято, кашель оценивать как легочный симптом, что и определяет круг основных подозреваемых заболеваний легких и последующий диагностический поиск. Однако, кашель может быть проявлением заболеваний не только бронхолегочной системы, но также заболеваний сердца, околоносовых пазух, желудка и некоторых других состояний.   

Исследования проведенные  в зарубежных, специализированных клиниках показали, что распространенными причинами хронического кашля являются множество причин, среди которых ведущая роль принадлежит астме (кашлевой вариант), гастроэзофагеальному рефлексу и ринитам. 

По результатам клинического исследования проведенного в феврале 2011 года у детей наибольшей причиной кашля, являются астма 40,5%, заболевания ВДП 30,6%, постинфекционный кашель 17,1 %. 

Хронический сухой или навязчивый кашель представляет большую проблему в диагностике. Диагностический поиск при наличии у больного кашля должен базироваться не только на информации об особенностях и характере кашля, но и на учете состояния различных органов и систем у данного пациента, а также других анамнестических и клинических данных. Диагностический алгоритм у больных с хроническим кашлем включает: сбор данных анамнеза, физикальный осмотр, рентгенография органов грудной клетки, общие лабораторные анализы (эндоскопическое обследование ЛОР-органов (особенно гортани). В помощь практическому врачу респираторные сообщества, в частности  Американская коллегия врачей-специалистов по заболеваниям грудной клетки (American College of Chest Physicians, ACCP), Европейское респираторное общество (European Respiratory Society, ERS), Британское торакальное общество (British Thoracic Society, BTS), создали специальные рекомендации по ведению пациентов с кашлем. В рекомендациях представлены сведения о распространенности, этиологии, диагностике, прогнозе, осложнениях и лечении кашля как на уровне первичного звена здравоохранения, так и в специализированных клиниках.

 Лечение хронического кашля должно быть комплексным и направлено на устранение причин болезни, а также ее симптомов и на повышение защитных сил организма. При проведении лечебно-диагностических  мероприятий следует учитывать является кашель острым или хроническим, продуктивным или сухим. Успешное лечение кашля является очень важной проблемой.  Так например, при назначении препаратов для лечения сухого кашля, практический врач  должен собрать тщательный анамнез по возникновению кашля. Как правило, во многих случаях, препараты для лечения сухого кашля назначаются на ранних этапах ОРВИ и инфекциях дыхательных путей, когда отмечается не сухой,  а малопродуктивный кашель, который через несколько дней заменяется продуктивным. В таких случаях, данная группа препаратов усугубляет состояние больных.  Препараты для лечения сухого кашля назначаются при остаточных явлениях перенесенных заболеваний дыхательных путей, как например  сухой,  навязчивый, кашель. Назначаемые при этом противокашлевые препараты центрального действия (угнетают кашлевой центр продолговатого мозга) могут использоваться в качестве монопрепаратов или входить в состав комплексных противопростудных препаратов.  Использование противокашлевых ЛС, преимущественно неопиоидных, может быть рекомендовано детям с сухим, раздражающим кашлем (как острым, как и хроническим). Следует учитывать, что маленькие дети очень чувствительны к использованию этих ЛС, и у них могут быть такие побочные эффекты, как сонливость, иногда заторможенность, даже при использовании соответствующих возрастных дозировок.  В педиатрии, при лечении сухого кашля возникает необходимость в назначении противокашлевых препаратов у которых отсутствуют вышеуказанные побочные эффекты.

Одним из таких  новых средств при лечении сухого кашля, является левоклоперастин.

Клиническая эффективность левоклоперастина (Privituss) при сухом кашле различной этиологии оценивалась и у нас.

Улучшение оценивалось субъективными (частота/интенсивность кашля, ночные  пробуждения, затруднение выдоха), объективными и лабораторными (респираторные/кардиоваскулярные) показателями. Оценивали в первый и в последний день лечения. Клиническая эффективность оценивали как -«отличный», «хороший», «удовлетворительный », «неудовлетворительный». При этом клиническая эффективность Privituss в обоих группах составляла отличный эффект у детей 87% и у 83 %  взрослых. Хороший эффект отмечался у детей в 8%, у взрослых в 12%. Удовлетворительный эффект  5% детей и 4 % взрослых. Неудовлетворительный эффект отмечался только во взрослой группе 1%, что связано с соответствующей патологией у больного.

 Исходя из вышесказанного, препарат Privituss является эффективным при лечении сухого кашля связанного с рядом острых и хронических респираторных заболеваний и хорошо переносится у пациентов всех возрастов.

Therapeutic Use of Levocloperastine as an Antitussive Agent

Abstract

Cough associated with acute and chronic respiratory conditions is common in patients of all ages. Levocloperastine is a novel antitussive with a pharmacological profile distinct from that of the racemic DL-cloperastine. Levocloperastine with its dual mechanism of action acts on both the central bulbar cough centre and on peripheral receptors in the tracheobronchial tree. In preclinical studies, levocloperastine demonstrated antitussive effects similar to those observed with codeine. In acute and repeated-dose toxicity studies, levocloperastine was well tolerated in rodents and dogs, with no clinically significant cardiovascular or gastrointestinal adverse events. The pharmacokinetic behaviour of levocloperastine, best described by a two-compartmental model with absorption phase, is similar to that of the racemic compound DL-cloperastine. In clinical trials, levocloperastine had a faster onset of action and produced greater reductions in the intensity and frequency of cough compared with DL-cloperastine, codeine and levodropropizine. The antitussive effects (reduction in intensity and frequency of cough) of levocloperastine were observed after the first day of treatment in patients of all ages. In children, levocloperastine reduced night-time awakenings and irritability; in adults, it was also effective in treating ACEinhibitor cough. Levocloperastine was generally well tolerated. There was no evidence of clinically significant central adverse events, whereas drowsiness, dry mouth and nausea were reported with comparator agents (levodropropizine, codeine, DL-cloperastine). Levocloperastine represents an effective alternative to currently used antitussive agents with the added advantage of faster onset of action and improved tolerability in all patient groups.

Introduction

Cough, a reflex phenomenon occurring when sensitive receptors located in the larynx and upper airways are activated, is one of the most common symptoms for which patients consult primary care physicians and pneumologists.[1] Coughing serves a number of functions, including acting as an indicator of an underlying illness and as an important defence mechanism to clear the airways. If it becomes ineffective, excessive or persistent it can profoundly and adversely affect a patient's quality of life.[2] Treatment of cough depends on the function the cough is serving. When cough indicates an underlying illness, the aim of therapy is to cure such illness, but treatment should also attempt to control, prevent or eliminate cough using antitussive agents.[3]

Levocloperastine (Privituss®;1 Aesculapius Farmaceutici), the levorotatory isomer of DLcloperastine, is a centrally active, non-opioid antitussive agent with a chemical structure and pharmacological profile distinct from that of the racemate. It is devoid of central antinociceptive activities, does not cause addiction or dependence phenomena, and does not interfere with cardiovascular or gastrointestinal functions.[4,5] The therapeutic efficacy of DL-cloperastine in reducing the intensity and frequency of cough is well established, with efficacy similar to that of codeine and superior to that of dextromethorphan.[6-9] In this overview, we examine the pharmacological profile of levocloperastine and its clinical efficacy and tolerability in the treatment of cough associated with a range of respiratory conditions in patients of all ages.

Levocloperastine in the treatment of chronic nonproductive cough: comparative efficacy versus standard antitussive agents.

Abstract

The medical and social impact of cough is substantial. Current antitussive agents at effective doses have adverse events such as drowsiness, nausea and constipation that limit their use. There is also recent evidence that standard antitussive agents, such as codeine, may not reduce cough during upper respiratory infections. Therefore, there is a need for more effective and better-tolerated agents. The efficacy of levocloperastine, a novel antitussive, which acts both centrally on the cough center and on peripheral receptors in the tracheobronchial tree in treating chronic cough, was compared with that of other standard antitussive agents (codeine, levodropropizine and DL-cloperastine) in six open clinical trials. The studies enrolled patients of all ages with cough associated with various respiratory disorders including bronchitis, asthma, pneumonia and chronic obstructive pulmonary disease. Levocloperastine significantly improved cough symptoms (intensity and frequency of cough) in all trials, and improvements were observed after the first day of treatment. In children, levocloperastine reduced nighttime awakenings and irritability, and in adults it was effective in treating cough induced by angiotensin-converting enzyme inhibitors. When compared with other antitussive agents, levocloperastine had improved or comparable efficacy, with a more rapid onset of action. Importantly, no evidence of central adverse events was recorded with levocloperastine, whereas drowsiness was reported by a significant number of patients receiving codeine. Levocloperastine is an effective antitussive agent for the treatment of cough in patients of all ages. It has a more rapid onset of action than standard agents with an improved tolerability profile

Abstract

Cough constitutes an impressive expression of the normal defense mechanisms of the respiratory system. Productive cough associated with catarrh is an important protective system for the lung because it favors the upward movement of secretions and foreign bodies to the larynx and mouth. Cough may also appear without bronchial secretions, as dry cough, which may be persistent when inflammatory disease is chronic or when, in the early stages of respiratory disease, bronchial secretions are not yet fluid. Sometimes bronchitis-induced cough does not significantly affect quality of life, whilst in other cases cough may become so intense as to impair daily activities severely, resulting in permanent disability. This type of cough is one of the most frequent reasons for seeking medical advice. The use of cough suppressants may be appropriate for reaching a precise diagnosis and when dry cough is persistent. Cloperastine has been investigated in various types of cough and, unlike codeine, has been shown to possess dual activity. It also acts as a mild bronchorelaxant and has antihistaminic activity, without acting on the central nervous system or the respiratory center. Here we review the preclinical and clinical evidence of the efficacy and tolerability of cloperastine.

Clinical expert guidelines for the management of cough in lung cancer: report of a UK task group on cough.

Abstract

Background

Cough is a common and distressing symptom in lung cancer patients. The clinical management of cough in lung cancer patients is suboptimal with limited high quality research evidence available. The aim of the present paper is to present a clinical guideline developed in the UK through scrutiny of the literature and expert opinion, in order to aid decision making in clinicians and highlight good practice.

Methods

Two systematic reviews, one focusing on the management of cough in respiratory illness and one Cochrane review specifically on cancer, were conducted. Also, data from reviews, phase II trials and case studies were synthesized. A panel of experts in the field was also convened in an expert consensus meeting to make sense of the data and make clinical propositions.

Results

A pyramid of cough management was developed, starting with the treatment of reversible causes of cough/specific pathology. Initial cough management should focus on peripherally acting and intermittent treatment; more resistant symptoms require the addition of (or replacement by) centrally acting and continuous treatment. The pyramid for the symptomatic management starts from the simpler and most practical regimens (demulcents, simple linctus) to weak opioids to morphine and methadone before considering less well-researched and experimental approaches.

Conclusion

The clinical guidelines presented aim to provide a sensible clinical approach to the management of cough in lung cancer. High quality research in this field is urgently required to provide more evidence-based recommendations.

1. Introduction

Cough is a common symptom in about 23-37% of general cancer patients and 47-86% of lung cancer patients [1]. The first author's data on 100 cancer patients assessed using the Memorial Symptom Assessment Scale from the beginning of cancer treatment to 3, 6 and 12 months showed a prevalence of 42.9%, 39.2%, 35.1% and 36.1% respectively, similarly to the experience of breathlessness, although less distressing than breathlessness [2]; these numbers almost doubled in the lung cancer subgroup analysis. Despite such high prevalence, the management of cough remains suboptimal, with little high quality evidence to guide practice. Much of the current practice on the symptomatic management of cough in lung cancer is experiential and primarily is geared around the use of oral opioids. Current guidelines on the management of cough are often broad and non-specific (suggesting difficulty in making any specific recommendations) and either focus on non-cancer respiratory illnesses with different pathophysiology from cancer-related cough, or provide broad reviews of generally poor quality studies [3-7]. Professional societies that have developed guidelines (non-cancer) include the [7].

ABSOLUTE CONFIGURATION OF  THE LEVOCLOPERASTINE FENDIZOATE

INTRODUCTION  

Cloperastine is a cough suppressant. There is one stereogenic center in this compound. It has two

enantiomers namely (R)-cloperastine and (S)-cloperastine. Cloperastine is a racemic mixture of equal quantities of two enantiomers. Cloperastine,  R- cloperastine,  S-cloperastine and their mixture have different pharmacological effects. However, the levocloperastine is thought to be more effective[1]. The levocloperastine has negative specific rotation, but one can not know the structure of levocloperas- tine only from the optical rotation. What is the correct configuration of levocloperastine? Levocloperastine is a novel antitussive agent believed to have antihistaminic, antiserotonergic, and musclerelaxant properties[2]. It is used in pharmaceutical compositions generally in the form of the salt of a great variety of pharmacologically acceptable acids, such as inorganic acids (chloride) and organic (fendizoate)[3]. Levocloperastine fendizoate (Fig. 1) is a

salt of levocloperastine (CAS RN 132301-89-4) with fendizoic acid (CAS RN 84627-04-3), and its Chemical Abstract index name is 2-[(6-hydroxy[1,1΄- biphenyl]-3-yl)carbonyl]-benzoic acid, compound with (-)-1-[2-[(4-chlorophenyl)phenyl methoxy]ethyl]- piperidine (1:1) (CAS RN 220329-19-1). From levocloperastine to its fendizoate salt, the specific rotation is still negative. The immission of organic anion to levocloperastine  did not change the rotary polarization. Therefore, the absolute configurations of levocloperastine and levocloperastine fendizoate

are identical, but the absolute configuration of the only one chiral carbon atom in these molecules is S (sinister) by Cahn-Ingold-Prelog rule not only in the CA (Chemical Abstracts) record but also on some of the websites[4, 5] up to now. Is it right? To our knowledge, single-crystal structure X-ray analysis is the best method to disclose a chiral configuration. N. Marubayashi[6] has determined the crystal structure of cloperastine fendizoate, but did not explain the sample to be enantiomeric or racemic. The space group given in reference 6 is  P 1 which is a centrosymmetric space group, and the Cl atoms are disordered (0.8:0.2) in two phenyl rings in the same molecule. The absolute configuration of the chiral carbon atom can not be gotten from reference 6. It is necessary to determine the chirality so as to guide the preparation and synthesis of levocloperastine fendizoate. Surprisingly, the absolute configuration of the chiral carbon atom of levocloperastine fendizoate is R(rectus) based on a series of analyses

including specific rotation, single-crystal X-ray, MS, IR, NMR and elemental analysis.
